Community Intermediate Care Team


The focus of the Community Intermediate Care Team is community based rehabilitation. The team prevents unnecessary admission to hospital and facilitates early discharge from hospital for patients who need a spell of rehabilitation to regain independence.

Managed by the Trust, the team is jointly staffed between health and the local authority. Referrals are made for over 18s, who are medically stable and require up to six weeks’ rehabilitation to improve their independence.

The team supports patients with many different needs, including those who have had surgery, suffered a fracturer, are recovering from an acute infection, or patients with an existing medical condition that flares up or deteriorates.

Based at West Park Rehabilitation Hospital, the team has in the region of 40 staff including Nurses, Occupational Therapists, Physiotherapists, Rehabilitation Technicians and Support Workers.

The creation of its early supported discharge service for stroke patients has developed better links between wards and the capacity team at New Cross Hospital.
